Hi folks - I am working on a project where I need the Database structure (tables / linkages etc.) of a Longitudinal Health Record.

I am looking for a database structure where I can capture

  1. Patient Data and Medical History
  2. Previous symptoms and diagnosis
  3. Care plans
  4. Tests
  5. Medication data

Does anyone have something you can share ?

If this is intended towards research, history of parental diagnosis would appear to be useful.

You may be interested in reaching out to Partners HealthCare, who have a large database of EHR / EMR. I worked very briefly on this data.

The All of Us project in the US is using OHDSI's OMOP. We’re switching to it for a research project and so far it’s been well structured and the concept ids should make training models on it much easier as the numeric conversion is up front.

Hey Robert - thanks! You may want to check your link. There's a bug in the URL parsing whereby, if there's a parenthesis after a URL, it's interpreted as being part of the URL. You can edit your post and add a space to avoid this.
