Disease To Omim Id Or Mesh Id

Is there any way to map some chunks of loosely defined diseases to OMIM IDs or MeSH IDs. I want some file (csv/tsv) for the same.

There is the Disease-ontology project, which contains references to OMIM and MESH. You can download the complete database in a OBO format, which contains synonym for some terms, and do loose text matching.

Check CTD's merged disease vocabulary (MEDIC), a curated resource mapping OMIM disease terms into the MeSH hierarchical vocabulary. You can the ontology in various formats in the downloads section of CTD (here is the link to the article introducing the resource).
