Drug-Disease Pairs To Describe Drug Indications?

Hi all,

I'm looking for a database that provides me with drug indications in the format of drug-disease pairs. As far as I know, DrugBank is not useful in this case because it gives ATC codes (which are difficult to map to disease terms, for instance, in MeSH terminology or ICD-10). PharmGKB is not useful either because it provides drug-disease pairs without specifying if the relationship corresponds to an indication or to an adverse event.

the Therapeutic Target Database haa a drug-disease mapping file : bidd.nus.edu.sg/group/TTD/filedownload.asp?file=flatfiles/drug-disease_TTD2013.txt

Apart from the links given above, check comparative toxicogenomics database (CTD) : http://ctdbase.org/ It gives the adverse effect (disease) associated with drugs/chemicals. The "direct evidence" ones have higher confidence.
