How do I edit chromosome names or delete some chromosomes in assembly (FASTA) and annotation (GFF/GTF) file?

I have an assembly/genome file in FASTA format and a corresponding annotation (GFF/GTF) file.

How can I change names of chromosomes or delete some chromosomes off? Is there any software for this?

What have you tried? Have you googled this question? This has been asked many, many times previously (both here and elsewhere).
