In RSEM.isoform.TMM.EXPR.matrix, some values for a set of 3 samples are like this:

0.596   0.782   0
2.173   0   0
4465.372    3953.622    5033.097

Why there is so much difference between samples and among different transcripts?

Biological heterogeneity or technical artifacts sich as batch effects. This really cannot be answered meaningfully without context.
